2Where do you provide services?
Lott Community Home Health Care provides home care services in the communities of both Manhattan and the Bronx.
3Can Lott Community Home Health Care help with medications?
Our Registered Nurses provide medication management and teaching. We can also report any side effects to your physician and assist you to obtain pre-measured medication packages.
4Can I change or discontinue the services my loved one receives?
You can change the number or type of service we provide your loved one whenever it’s necessary. We understand the care situation can quickly change, so we’re as flexible as possible with your loved one’s care. Home Health Care services are voluntary, so you can discontinue your relationship with Lott Community Home Health Care at any time.
5Do I need a physician's authorization for your services?
Yes. In order to receive Home Health Care services, you are required to be under the care of a physician. In addition, your physician must agree to the services.
6Are home care services covered by insurance or Medicare?
Most home health care services are covered by insurances such as Medicare, Medicaid and/or managed care health plans. Lott Community Home Health Care will assess your eligibility for services under your insurance and will bill these payers directly for home health services. If you have more than one insurance, Lott Community Home Health Care will bill your secondary insurance for any uncovered items.
7How is the privacy of my personal information maintained?
We adhere to all applicable federal and state guidelines related to privacy of personal and healthcare information. The written Facts About Your Homecare and Consent Forms stipulates the exact restrictions on the use of your information. In fact, only the office personnel and clinical caregivers who are directly involved in your services will have access to information related to the care of your loved one.
